Your medical card

  • No trip to the DMV. Dr. Noah submits your medical card to the FMCSA National Registry, and the registry and the DMV are connected.
  • If there's a problem with your card, contact Hudson or call the CDL medical card line at (609) 292-5151. The new system started February 23, 2026, so some records may take a little time to transfer.

The exam

What the exam covers.

Every DOT physical follows the same federal standards, so you know what to expect before you walk in.

  1. 1

    Your health history

You fill out the driver's section of the federal Medical Examination Report form, then go over it with Dr. Noah.

  2. 2

    Vision and hearing

The standard is 20/40 in each eye, with or without glasses, and hearing a forced whisper at five feet, with or without a hearing aid. You'll also be asked to tell red, green and amber apart.

  3. 3

Blood pressure, urine test and exam

A blood pressure and pulse check, a physical exam, and a urine test that checks for protein, blood and sugar. The urine test is not a drug test.

How long your card lasts

A DOT medical certificate is good for up to 24 months. The examiner can certify you for a shorter time when a condition like high blood pressure needs to be watched.

Standards from the Federal Motor Carrier Safety Administration (49 CFR 391.41). Read more at FMCSA.
